How Cystic acne differs from Nodular Acne?

Both; Cystic and Nodular Acne are very painful and severe conditions of skin and both are deep located too,  but they have some distinguishing features. How the cysts are formed?

These sac like, lumpy structures, are formed when the hair follicle ruptures and the oily debris leaks into immediate vicinity. Soon after leakage it is enclosed by a membrane. Nodules on the other hand are dry and solid, dome shaped, bumpy structures which are smaller than cystic lesions. They are formed by rupturing of the hair follicle whereupon their infective content infects the adjacent hair follicles too. Sometimes both types co exist ; known as ‘Nodulocystic Acne’.

What causes Cystic Acne?

Two major causes of Cystic Acne are hormonal imbalance and microbial infection. Regarding hormonal imbalance, we know that it activates sebaceous glands which produce ‘Sebum’ or the skin oil in access. When a sebaceous gland duct is blocked for any reason, like the deposit of cellular material, the walls of sebaceous glands rupture and the infected sebum leaks out in the immediate vicinity and wrapped in a membrane.

This liquid filled sack, on account of its deeper location, can not rupture the outermost layer of skin, causing a pressure on the adjacent cells and nerves, which is very painful. The other major cause is bacterial infection is infection by Propionibacterium acne. It is mostly the causative organism, but sometimes the infection is also caused by Staphylococcus aureus, which is a usual resident (commensals) on human skin.
